Mind in Oxford Floating
Support
What help do we offer? We provide time limited housing related support for people with mental health problems who need short to medium term support. Our service is available to clients for less than two years. We support people to live independently. We do this by: 1. Assisting people to move onto appropriate housing 2. Helping people to keep their current accommodation 3. Working to prevent homelessness 4. Ensuring people get the help they need from other services We offer support: 1. To help people to respect their tenancy conditions 2. To claim benefits 3. To manage money and resolve debts 4. To help with getting used to a new home 5. To manage neighbour disputes 6. To signpost to other services You can use the service if: 1. You are an adult (16+) 2. You are on Enhanced Care Programme Approach (CPA) 3. You are in need of housing related support and willing to accept support 4. You live in Oxford City We cannot accept people who present a significant risk to staff. All decisions will be based on the ability of the service to manage these risks. Who can refer? We take referrals from: 1. Specialist mental health housing support services (including supported housing) 2. Community mental health teams 3. Homelessness support services 4. Oxford City Council How do I make a referral?
